Please be sure to read the Oral and Poster instructions prior to completing your Abstract Form.

IMPORTANT DATES
- January 1, 2015: Deadline for Oral Abstract Submission
Submission for oral presentation is OPEN. Colleagues are encouraged to submit abstract papers for Oral, Poster or Graduate Student presentations.

- February 6, 2015: Deadline for Graduate Student Abstract Papers
IMPORTANT: Graduate students or students graduated in the last 3 years are invited to submit papers for presentation. Acceptance of paper is based on decision making process involved in arriving at a diagnosis and appropriate treatment plan, taking into account the best available evidence. Prepare you treatment presentation abstract paper accordingly.

- March 10, 2015: Deadline for Poster Abstract Submission
IMPORTANT: Authors are solely responsible for the delivery of their abstract paper. If after posting your abstract paper on the above Abstract Form, if you did not receive a confirmation email... contact the ICP administration office.

- April 17, 2015: Selection of Oral Abstracts
Abstracts will be reviewed for approval and posted on the ICP Program Schedule. Authors should review the ICP conference Program Schedule to ensure their presentation has been accepted.
IMPORTANT: If you have not received correspondence from the ICP office or do not find your name or presentation title on the Program Schedule after this date, we do not have your paper and have not made arrangements for your presentation. If this is the case, authors must contact the ICP office to confirm participation.

- May 29, 2015: Oral Presenter Registration
All oral presenters must register and pay conference fees. Failure to register may forfeit your opportunity to present, as other colleagues are waiting for the opportunity.

- May 29, 2015: Early Discount Registration Fees
Save money and register early. Fees will increase after this date.

- May 29, 2015: Poster Presenter Registration
All poster presenters must register and pay conference fees. Failure to register may forfeit your opportunity to present, as other colleagues are waiting for the opportunity.

- July 31, 2015: Printing Deadline
This is the last date to edit abstract papers. Abstracts will be printed as submitted by author.
